Booking Appointments at the Foot & Ankle Clinic at Lindsay Park
We would like to provide you with some information and a downloadable form that might be helpful when referring patients to the Foot & Ankle Clinic.

To assist in expediting patient care, a referral letter is required. The letter should include:
- Reason for referral
- Patient name, telephone number and date of birth
It is important that patients be instructed to bring with them:
- Recent X-rays (films, not just reports)
- A list of current medications
- Health card
Please also ask patients to contact the clinic if they cannot attend their appointment.
Keeping the Referring Physician Informed
We are sensitive to the relationship between referring physicians and their patients. We understand your concerns about your patient's health, and we will keep you informed through every phase of your patient's care.
- Following your patient's initial consultative visit with Dr. Haverstock you will receive a letter outlining his findings and treatment plan.
- Update on any significant findings (MRI, CT Scan, Labs).
- For patients undergoing surgery at an outpatient surgery centre or the hospital, Dr. Haverstock will request a consultation from the patient's family physician to complete a pre-operative medical history and physical examination.
